Overview
Rheumatoid Arthritis (RA) is a chronic, progressive, and disabling multi-system disease characterized by pain, swelling, and stiffness in the synovial joints. This stiffness is often worse in the morning or after periods of inactivity. RA typically presents as symmetrical, destructive, and deforming polyarthritis affecting both small and large synovial joints. It can also lead to extra-articular manifestations such as fatigue, subcutaneous nodules, lung involvement, pericarditis, peripheral neuropathy, vasculitis, and hematologic abnormalities.
In the Siddha system, this condition is known as Uthira Vatha Suronitham or Vali azhal Keel vayu. It is understood as a chronic and systemic inflammatory arthritis primarily affecting tissues and organs due to an escalation of the Pitham (bile) humor along with Vatham (wind). The disease is marked by pain and swelling in joints like the ankles, knees, and fingers, often accompanied by dizziness and loss of appetite.
Siddha medicine attributes the pain and redness associated with Uthira Vatha Suronitham to factors such as the excessive consumption of Vatha-aggravating foods like potatoes, mutton, fish, and eggs, as well as undigested food products. Other contributing factors include exposure to cold air and rain, frequent alcohol consumption, and a lack of physical activity. These factors can lead to undigested food, causing symptoms like belching, flatulence, constipation, and obesity.

- 🌡️ Dizziness.
- 🌡️ Anorexia (loss of appetite).
- 🌡️ Swelling in both knee joints, wrist joints, ankle joints, and dorsum of feet.
- 🌡️ Severe pain in minor joints.
- 🌡️ Patient endures distress.
- 🌡️ Patient loses interest in oral intake.
- 🧠 Stiffness and swelling in minor and major joints (Sparisam finding).
- 💧 Straw or hay coloured urine (Neerkuri finding).
- 💧 Oil may spread in the form of pearl/ring/snake pattern indicating Kabam/Pitham/Vatham humours (Neikuri finding).

Therapeutic Approaches in Siddha
Siddha treatment for Rheumatoid Arthritis involves a multi-pronged approach including internal medicines, external applications, and procedural practices. Internal treatments focus on balancing the vitiated humors, particularly Vatham and Pitham, through various formulations like Kudineer, Churanam, Chenduram, Mathirai, Ilagam, and Mezhugu. These preparations utilize a range of herbs and mineral compounds aimed at reducing inflammation and pain.
External applications are crucial for localized relief and detoxification. These include medicated oil baths (Ennai muzhukku) using oils like Chukku Thailam or Seeraga Thailam, and poultices (Kattu) made from ingredients such as Pirandai. Medicated oils are also applied externally for massage and as part of specific treatments like Poochu.
Procedural practices are integral to the Siddha management of Rheumatoid Arthritis.
