Overview
Rheumatoid arthritis (RA), known in Ayurveda as Amavata, is a chronic, systemic inflammatory autoimmune disorder. It primarily affects the synovial joints, causing pain, swelling, and stiffness, often worse in the morning or after periods of inactivity. This condition can also impact other organs and tissues.
Ayurveda views Amavata as arising from a hypo-functioning of digestive fire (Agni), leading to the formation of 'Ama' – a toxic, undigested substance. This Ama, combined with vitiated Vata and other doshas, circulates through the body's channels and lodges in the joints, causing inflammation and symptoms. Factors contributing to this include incompatible food and lifestyle habits, poor digestion, and a sedentary lifestyle.
RA commonly affects females more than males, with a higher prevalence in individuals over 55 years old. Risk factors include genetic predisposition, environmental exposures like tobacco smoke and pollution, and dietary habits such as high sodium, red meat, and iron consumption, alongside obesity and low vitamin D levels.

Therapeutic Approaches in Ayurveda
In Ayurveda, the treatment of Amavata (Rheumatoid Arthritis) focuses on addressing the accumulation of 'Ama' (toxins) and the vitiation of Vata dosha. Internal treatments often involve procedures like Langhana (regulated fasting) and the administration of substances that aid in 'Ama Pachana' (digestion of toxins) and 'Deepana' (appetite stimulation). Shamana Chikitsa, which includes the use of bitter and pungent substances, is also a key component.
External therapies play a significant role in alleviating pain and inflammation. These include Abhyanga (therapeutic oil massage) on affected joints with specific medicated oils, Swedana (sudation or fomentation) using dry or medicated substances, and the application of Lepa (herbal pastes) or Upanaha (poultices). Procedures like Kashaya Dhara (continuous pouring of warm herbal decoctions) and Pichu (application of medicated cotton swabs) are also employed.
Bio-purification procedures, known as Shodhana Chikitsa, are considered crucial for eliminating deep-seated toxins. Virechana (medically induced purgation) and Basti Chikitsa (medicated enemas) are the primary procedures utilized. These are carefully selected and administered based on the individual's constitution and the specific stage of the disease, aiming to restore balance and promote healing.
